Telomeres are known to be one of the most critical factors affecting cell life and death. Dysfunction of the telomere may lead to the development of a number of malignancies and their metastasis. In this study, we examined the potential prognostic significance of the length of telomeres in primary breast cancers. DNA was extracted from 113 female breast cancer tissues and matched adjacent normal tissues. The telomere repeat length (TRL) in these tissues was measured using a TRF assay. The relationships between the TRL and clinicopathological variables, especially patient prognosis, were analyzed. The length of telomeres in tumor tissues was significantly shorter than in normal tissues (p=0.000), and TRL was significantly shorter in patients with tumors that were 2 cm or more in size (p=0.000). The 5-year disease-free survival rates were significantly lower in patients with TRLs of 9 or less than 9 kbp, compared to the rates in patients with TRLs of 10 kbp or more (p=0.001). In a multivariate analysis, we found that the TRL was associated with a higher risk of recurrence in postmenopausal women (p=0.049), and patients with small tumors (p=0.020) and short TRLs (p=0.038) had a higher risk of death from disease. Patients with tumors of 4 cm or less in size who had a TRL of 9 or less had a higher risk of recurrence (p=0.032) and patients with a TRL of 9 or less were more likely to die of their disease (p=0.042).
